Job Description

The Care Coordinator is assists all patients through the system by acting as a patient advocate and navigator.

  • Works closely with school social workers and nursing staff to coordinate care
  • Meets with the patients and parents to coordinate care
  • Communicates with parents via telephone and in person to gain appropriate consent when needed
  • Performs data collection pertaining to SBHC state regulations
  • Maintains registration lists and authorization of treatment for all services provided within the SBHC
  • Coordinates with IC Primary Care clinics for other patient and administrative needs.  
  • Receives/reviews referrals from provider after patient's initial visit
  • Facilitates communication with the referral provider
  • Educates patients and or parents on insurance and self-pay process
  • Arranges future appointments with referral providers as needed
  • Ensures all appropriate paper work is received by referral provider
  • Ensures adequate transportation is arranged for referral appointments
  • Follows up with patient and or parents approximately a month after initial referral to ensure that appointments are being kept.       Determines patient’s needs to meet the follow-up plan if appointments are not kept.
  • Contacts referral provider to obtain records of office visits, labs etc.
  • Determines if further follow-up care is required and coordinates with patient and referral partner for next steps.

Job Requirements

Knowledge of case management principles and procedures; knowledge of and ability to apply quality standards, procedures and protocols; knowledge of social, cultural, economic, medical, psychological and legal issues which influence attitudes and behaviors of patients and families; knowledge of and ability to apply clinical therapeutic models of client care with emphasis on interdisciplinary team approach; ability to work in a cooperative and collaborative manner as a team member with agency associates as well as external teams and family members; strong interpersonal skills and oral, written and computer communication skills; ability to learn specialized client care applicable to work assignment; ability to listen and provide empathetic assistance to clients and families in meeting their needs and carrying out prescribed treatments; ability to develop therapeutic relationships with clients, and their caregivers; ability to respond to emergencies; ability to respond to and seek supervision; ability to analyze and evaluate proper care and treatment.

A Bachelor’s degree in a health care field and two (2) years of experience in a healthcare setting. Extensive community health experience may be substituted for the degree requirements noted above.
