We are currently recruiting a full-time COMMUNITY HEALTH WORKER in our new Transitions Clinic located in Hartford.  The CHW will be responsible for helping clients that are returning from correctional facilities and are in need of primary care. As a priority, the CHW will promote, maintain and improve the health of clients. They will provide social support and advocate for individuals and community health needs.

 

Job Requirements

  • Responsible for establishing trusting relationships with clients and their families while providing general support and encouragement.
  • Works with other staff of InterCommunity and clients to coordinate services to facilitate recovery and acts at the Point of Contact for each referring agency.
  • Provides assistance in identifying, with the individual, risk factors related to psychiatric and/or substance abuse disorder relapse and strategies to prevent relapse.
  • Provides assistance and support for the individual in crisis situations; coordination and/or assistance with crisis providers as needed.
  • Provides education, support and consultation to individual’s families and their support system which is directed exclusively to the well being and benefit of the individual, if needed.
  • Provide linkages with various community resources..
  • Completes all required documentation related to treatment in accordance with established procedures.
  • Complete all required reporting standards to various service providers and referral sources in accordance with timelines set forth in program standards.
  • Conduct intake interviews with clients and will work with clients collaboratively to identify required services by assessing the overall needs of each client. Assist clients with completing applications and registration forms.
  • Responsible for ensuring that the client is actively involved in the service(s) he or she requires to remain in the community, arrange for needed services, regularly check on the client’s progress, monitor the impact of these services and advocate on behalf of their clients and reports information to referral agencies.
  • Follow up with clients via phone calls, home visits and visits to other settings where patients can be found.
  • Performs related duties as required.

Ability to initiate and maintain positive working relationships with staff and other organizations. Understand the community served- community connectedness. Ability and willingness to provide emotional support, encouragement and motivation to clients. Formerly incarcerated and 24 months since time served required.

High School Diploma or Equivalent. Experience in a multi-cultural setting. Experience working in a community-based setting for at least one (1) to two (2) years preferred. Knowledge of some medical terminology preferred. Basic computer skills. Considerable oral and written communication skills
