Job Description

We are currently recruiting a full-time, third-shift HOUSING CASE MANAGER/DATA ENTRY CLERK.  This individual may have infrequent contact with our clients within a supported/supervised living program but the overwhelming majority of the position will be data-focused.

Essential Duties and Responsibilities:

  • Implements programs and monitors clients within a supported/supervised living program
  • Functions as a member of an interdisciplinary team and participates in service delivery plan process for clients
  • Provides guidance and support for clients to develop independent living skills
  • Participates in and assists clients with activities of daily living
  • Assists clients in accessing goods, services and supports required for independent living; reinforces appropriate client behavior through modification techniques
  • May provide crisis management for client’s to ensure client’s health and safety
  • Observes and reports client behavior
  • Serves as advocate for government subsidies and re-determination hearings
  • Transports clients
  • Acts as liaison to clients and their families or various community agencies or organizations.
  • Provides concise and accurate reports and documentation
  • May make recommendations on policies or standards
  • May administer first aid in emergency situations
  • Provides data entry for multiple programs into several sites
  • Performs related duties as required
