We are currently recruiting a full-time CARE COORDINATOR for our Behavioral Health Home, an integrated healthcare service delivery model that is recovery-oriented and person- and family-centered.

Job Requirements

To be considered for this position, an applicant must meet the following requirements:

  • A Bachelor’s Degree in Social Work strongly preferred.  A Bachelor’s Degree in a related Human Services field or equivalent Social Work experience will be considered.
  • A thorough knowledge of mental health issues and the services, programs and entitlements available in Connecticut
  • Bilingual (English/Spanish)
  • Strong interpersonal, oral and written communication skills
  • Ability to work effectively in a team-based environment
  • An adaptive/flexible work style
  • Current computer skills/ability to type efficiently

Strong candidates should also possess:

  • Behavioral health experience a plus*
  • The ability to provide client-centered services
  • Knowledge of Recovery Oriented Behavioral Health in Connecticut
  • A belief that every individual in need deserves quality service and an opportunity for recovery
  • A commitment to professional growth and development as well as to organizational growth
